Hospital systems have traditionally used a "wall of silence" or "deny and defend" approach to events rather than a collaborative "communication and resolution" program.  Three reasons for this is lack of training, fears of lawsuits, difficulty of discussions. 

Most providers have not been trained in safety event management. The idea of reporting events frightens many providers who fear malpractice and career harms, but reporting is crucial to safety. Providers are often lacking in skills to empathetically communicate after events, mitigate harm, and to participate in root cause analysis. Further, many lack skills in recognizing provider distress. Increased skills in all these areas improves system learning and patient safety and mitigates provider and patient harm after events.  These programs also reduce malpractice expenses globally.

Target Audience

Physicians, NP, PA, RNs and allied fields from pharmacy to other clinicians to management.

Learning Objectives

  • Incorporate harm communication planning and delivery skills.
  • Participate fully in event reporting and positive safety culture.
  • Demonstrate ability to participate in event review.
  • Identify effective and ineffective methods for system change.
  • Discuss possible barriers and biases which may impact patient care (i.e., race, ethnicity, language, gender identity/orientation, age, socioeconomic status, attitudes, feelings, or other characteristics).
