2026 Endocrinology Grand Rounds | The Evolution of Thyroid Cancer Diagnosis: From Initial Evaluation to Artificial Intelligence
The diagnostic landscape for thyroid nodules and thyroid cancer continues to change as new evidence, molecular diagnostic tools, and computational approaches become available. Clinicians must determine how these emerging technologies complement rather than replace clinical judgment, ultrasound findings, cytology, and established diagnostic pathways. A focused Grand Rounds presentation by an expert in thyroid nodule and thyroid cancer diagnosis will provide learners with an evidence-based framework for evaluating these evolving approaches and their appropriate incorporation into patient care. The educational need is particularly timely because the American Thyroid Association (ATA) recently published updated 2025 differentiated thyroid cancer management guidelines.
The evaluation and diagnosis of thyroid nodules and thyroid cancer have evolved substantially with advances in ultrasound risk stratification, cytologic assessment, molecular testing, and increasingly sophisticated diagnostic technologies. Clinicians may have variable familiarity with how newer diagnostic approaches should be integrated with established clinical and imaging assessment to accurately estimate malignancy risk and guide individualized management. It is imperative to translate emerging evidence and evolving diagnostic technologies, including artificial intelligence, into evidence-based clinical decision-making while avoiding unnecessary testing and procedures.
Target Audience
Endocrinologists, endocrine fellows, physicians, advanced practice providers, and other healthcare professionals involved in the evaluation and management of thyroid nodules and thyroid cancer.
Learning Objectives
- Outline the evolution of diagnostic strategies for thyroid nodules and thyroid cancer, including clinical assessment, imaging, cytology, and molecular testing.
- Apply contemporary risk-stratification principles to determine appropriate diagnostic evaluation of patients with thyroid nodules and suspected thyroid malignancy.
- Assess the clinical utility and limitations of molecular diagnostic testing in patients with indeterminate thyroid nodules.
- Examine emerging applications, potential benefits, and limitations of artificial intelligence in thyroid nodule and thyroid cancer diagnosis.
- Discuss possible barriers and biases which may impact patient care (i.e., race, ethnicity, language, gender identity/orientation, age, socioeconomic status, attitudes, feelings, or other characteristics).
