2024 Endocrinology Grand Rounds | Disorders of Alkaline Phosphatase Affecting Bone Metabolism: Hypophosphatasia and Paget's Disease of the Bone

Hypophosphatasia (HPP) is a rare metabolic bone disease that is not encountered in daily practice, and as a result, can be easily missed. Additionally, HPP has multiple classifications, and varies in its manifestation. Symptoms differ by age, and if left untreated, has a poor life prognosis. In contrast, Paget's disease of the bone is a common metabolic bone disease, just second in line to osteoporosis, however, it too is more common with age and discovery of it can vary, as it can be asymptomatic. 

Practicing clinicians need to be kept up to date and abreast of latest advancements in the evaluation and management of metabolic bone disease such as hypophosphatasia and Paget's disease affecting serum alkaphine phosphatase at different spectrums during evaluation of osteoporosis to identify the causes of osteoporosis and offer treatment accordingly.
 

Target Audience

Faculty and Staff of department of Diabetes, Endocrinology and Metabolism.

Learning Objectives

  • Examine the two causes of bone fragility, other than osteoporosis.
  • Outline the disease manifestation of these two conditions that are on opposite spectrum of alkaline phosphatase level. 
  • Recognize both subtle and more severe variants of HPP and Paget's disease, and master their management.
  • Discuss possible barriers and biases which may impact patient care (i.e., race, ethnicity, language, gender identity/orientation, age, socioeconomic status, attitudes, feelings, or other characteristics).
