2025 Endocrinology Grand Rounds | Current Strategies for the Diagnosis and Management of Cushing's Syndrome

Cushing's disease is relatively rare and sometimes may not present as typical symptoms such as weight gain, facial fullness or even their discriminatory ones like easy bruising, proximal myopathy, etc. Although this disease is unmistakable when full blown, because the clinical presentation is broad, practicing clinicians need to be kept up to date and abreast of the latest advancements in the evaluation and management of it.

There have been many advances in the field of Cushing's Syndrome, particularly new options of medical treatment. However, the rarity of this disease, the high incidence of other conditions like depression obesity, etc., coupled with limitations in screening, can increase the opportunity for false-positive results. As a result, it is necessary that clinicians stay up to date on the latest standards of care in this field.

Learning Objectives

  • Identify the clinical presentation of Cushing’s Disease – both classic and atypical manifestations.
  • Explore diagnostic strategies, including key tests used to confirm Cushing’s syndrome and differentiate adrenocorticotropic hormone (ACTH)-dependent from ACTH-independent cases.
  • Outline factors that influence test results, including cyclic Cushing’s, comorbid conditions, and medications.
  • Examine advanced diagnostic options such as inferior petrosal sinus sampling and stimulation/suppression testing.
  • Distinguish surgical, medical, and radiation-based therapies, with a focus on medical treatment options and their mechanisms.
  • Discuss possible barriers and biases which may impact patient care (i.e., race, ethnicity, language, gender identity/orientation, age, socioeconomic status, attitudes, feelings, or other characteristics).
