2026 Cardiology Grand Rounds | Diagnosis and Treatment of Transthyretin Amyloid Cardiomyopathy
Transthyretin amyloidosis (ATTR) is a massively under-recognized disease, and its symptoms are not very specific. Patients may go years without diagnosis, as this disease is often hiding within other more common syndromes. It is imperative to improve recognition of diagnosis and treatment of this condition, which may go undetected by healthcare providers.
Increased diagnosis of Transthyretin amyloidosis (ATTR) will improve morbidity and mortality. There are multiple effective emerging treatment strategies. Population-based analyses and systematic reviews demonstrate that diagnosed prevalence is considerably lower than expected from prospective screening studies, which identify ATTR-CM in approximately 10–15% of several high-risk populations (e.g., older adults with HFpEF, unexplained LV hypertrophy, or severe aortic stenosis). These findings support a significant gap in disease recognition and underscore the need for increased clinician awareness and earlier diagnostic evaluation.
