2026 Endocrinology Grand Rounds | Cushing’s Syndrome due to Ectopic Adrenocorticotropic Hormone (ACTH) Secretion: A Clinical Challenge
Ectopic Adrenocorticotropic Hormone (ACTH) mediated Cushing's syndrome is a rare and challenging disease. It is difficult to distinguish between ACTH mediated Cushing's from Cushing's disease. Not all clinicians are familiar with the entity, leading to diagnostic delay.
Given the rarity of ectopic ACTH mediated Cushing's syndrome, clinicians may have limited exposure and clinical experience to this disease. This may lead to the clinician's lack of confidence and ability to take care of patients affected by this rare disease. However, given the increased risk of morbidity and mortality in patients with ectopic ACTH mediated Cushing's syndrome compared to other forms of Cushing's, it is important that clinicians become comfortable in diagnosing and managing ectopic ACTH mediated Cushing's syndrome. This activity will enhance the endocrinologist's ability to care for this unique population alongside their oncology colleagues by discussing how to diagnose ectopic ACTH mediated Cushing's utilizing available diagnostic test and imaging modality and how to best manage ectopic ACTH mediated Cushing's.

Learning Objectives
- Identify the most common tumors associated with ectopic adrenocorticotropic hormone (ACTH)-mediated Cushing's syndrome.
- Recognize features that distinguish ectopic ACTH-mediated Cushing's syndrome from other forms of Cushing's diseases.
- Utilize diagnostic tests and imaging modalities to diagnose ectopic ACTH-mediated Cushing's syndrome.
- Identify the most appropriate treatment plan for patients with ectopic ACTH-mediated Cushing's syndrome.
- Discuss possible barriers and biases which may impact patient care (i.e., race, ethnicity, language, gender identity/orientation, age, socioeconomic status, attitudes, feelings, or other characteristics).
