Coordinated Co-Management in the Duarte ICU: Reflection on the First Five Years

Duarte, CA US
November 19, 2025

There is currently a great deal of heterogeneity throughout the City of Hope enterprise with regard to ICU rounding structure and critical illness care models.  As the COH enterprise continues to expand both in size and scope, consistency of ICU care will become increasingly critical.  This activity is intended to promote quality and consistency throughout our larger network by discussing and promoting the collaborative model which we have implemented in Duarte with tremendous success.

With the exponential growth of the City of Hope enterprise in recent years the need for consistency and uniformity in patient care models has become increasingly important.  The collaborative multi-disciplinary ICU rounding structure that we have pioneered on the Duarte campus over the past five years has subsequently become the standard of care for critical care services at City of Hope.  This educational activity is intended to promote increased utilization of this model throughout the larger enterprise including Lennar as well as the CAP sites.

Target Audience

This activity is intended to address the educational needs of patient-facing City of Hope practitioners who are involved in or concerned with the care of ICU patients within the City of Hope network.

Learning Objectives

  • Examine the applicability of a coordinated co-management model in ICUs enterprise wide
  • Assess barriers to implementation of a coordinated co-management in local ICUs
  • Integrate the core tenets of a coordinated co-management model into local ICUs
  • Discuss possible barriers and biases which may impact patient care (i.e., race, ethnicity, language, gender identity/orientation, age, socioeconomic status, attitudes, feelings, or other characteristics).
